Criminal Background Check

Many of the clinical sites where students complete their hands-on clinical experiences (such as hospitals, clinics, and other agencies) require verification of criminal history check before students can be placed in their organization. Based on the requirements of these agencies, as well as state and federal regulations, all students applying for admission to the Bachelor of Science in Nursing (BSN) degree programs must provide a limited criminal history check to NU 122, Center for Academic Affairs by the application deadline date.

A new criminal history check must be submitted each time a student applies to the School of Nursing. This requirement does not have to be met by students in the RN to BSN program.

Please note: Applicants who have been a resident in another state for six months or more at the time of application must provide a criminal history check from that state to NU 122, Center for Academic Affairs by the application deadline date.

A student's past criminal history may affect his or her admission to the BSN programs, limit the number of suitable practice placement sites, and/or affect ability to obtain licensure as a registered nurse (RN) in the state of Indiana and other states.

If a student's criminal history changes, it is required that the student notify one of the academic counselors located in the Center for Academic Affairs in NU122. Failure to do so may be grounds for dismissal.

Instructions
  1. Students should submit an original of State Form 8053, Request for Limited Criminal History Information, as part of their application for admission.
  2. Print the form and complete the personal information at the top of the form. Under "reason for request," check the first box as though you are applying for a job. Also, indicate that you are the "requestor" and have the report sent to you.
  3. Submit the form, along with a cashier's check or money order in the amount of $7 made payable to the State of Indiana and a self-addressed, stamped envelope, to:

When you receive your criminal history report from the Indiana State Police, mail or hand deliver one copy to:
